Chapter 25

Mia and Adam set off before sunrise. They had a four-hour drive ahead of them, and Mia wanted to arrive at the village of Destone before noon.

As soon as the car left Brewick and joined the major road, Mia fell asleep. When she next opened her eyes, the landscape had completely changed. She sat upright, casting her eyes across the terrain. Gone were the scenic granite houses and cobblestone streets that had brought memories of Aberdeen to the fore. Instead, Adam was driving through a town that at first glance seemed impoverished.

The houses were a mix of terraced buildings and apartment buildings. The facades of many showed signs of neglect and disrepair, broken or boarded-up windows, damaged roofs, and overgrown gardens. Graffiti tags and offensive messages defaced walls and fences. Bins overflowed with rubbish that spilt out of the containers and onto the street.

Mia could see several people loitering outside some of the buildings and on the street corners with no apparent purpose or activity. Adam’s car had attracted attention, and Mia observed more than a few eyes tracking it as they drove past.

“I’m not sure whether to be relieved or appalled that anti-social behaviour exists in other worlds too,” Mia commented.

She recalled how much she dreaded going on home visits to areas such as this one—places where drugs and addicts were rife and where she could easily step on a used needle in the hallways if she did not keep her eyes open. Which was difficult, considering that most hallways in such buildings were dimly lit or not lit at all. She always ensured that the brightly coloured doctor’s lanyard, in capital letters and reflective ink, was prominently displayed around her neck. This usually did the trick and protected her from harassment. Even so, Ham had never stopped worrying that Mia might come to harm someday.

Ironically enough, she had not come to harm at the hands of young drug addicts, but rather an old, kindly, harmless couple.

General practitioners wouldn’t normally be called out to see young or middle-aged people, but elderly people, often housebound, also lived in such areas. Many were retired drug addicts or ex-prisoners who had been released from prison after a long stint, but some were simply unfortunate people with no pension and no family, who had no choice but to live there.

Mia had once had to request a police escort when she was called out for a visit, but the police call handler had strongly advised her not to go to that particular building. Instead, she had been told to request the paramedic and police team to go out jointly and directly admit the patient to the hospital.

Mia had also worried that her car might permanently change ownership between the time she entered the building and when her visit was done—or, if it was still there, it would be missing something vital. Such as a converter, four tires, or an entire engine.

She had attended quite a few seminars about the inverse care law, the idea that people who needed medical care the most were least likely to receive it.
